If you plan on using dental insurance – you must bring your insurance card, or;

If you do not have a card, the following information will be needed:


Subscriber information: Name, Birthdate, Social Security Number, and Insurance ID number


Employer Information: Name of Company


Insurance Company Information: Name of Company, Address (*important), Phone Number


Patient Information: Insurance ID Number or Social Security Number, Group Number


Please make sure your information is for your Dental and not your Medical Insurance.

We need this information to properly file a claim on your behalf.
